Avery Edwards Impact Camp Participant Waiver, Release of Liability & Assumption of Risk By registering for and participating in the Avery Edwards Impact Camp presented by Home Team Apparel, the participant and/or parent/legal guardian acknowledges and agrees to the following terms and conditions: Assumption of Risk Participation in athletic activities, including football drills, competitions, fitness activities, conditioning, and related camp events, involves inherent risks including but not limited to falls, collisions, physical contact, heat related illness, equipment failure, serious bodily injury, permanent disability, illness, paralysis, and death. The participant and parent/legal guardian voluntarily assume all risks associated with participation in the Avery Edwards Impact Camp and related activities, whether known or unknown. Medical Acknowledgment The participant and parent/legal guardian certify that the participant is physically able to safely participate in camp activities and has no medical condition that would prevent participation. The participant and parent/legal guardian understand that camp staff, organizers, volunteers, sponsors, coaches, and affiliates are not responsible for determining medical fitness for participation. In the event of injury or medical emergency, the participant and parent/legal guardian authorize camp staff to obtain emergency medical treatment if necessary. Any medical expenses incurred shall be the sole responsibility of the participant and/or parent/legal guardian. Release of Liability The participant and parent/legal guardian hereby release, waive, discharge, and hold harmless Avery Edwards Impact Camp, Home Team Apparel, Avery Edwards, camp staff, coaches, volunteers, sponsors, vendors, facilities, event partners, affiliates, and any associated individuals or entities from any and all liability, claims, demands, causes of action, damages, costs, expenses, or compensation arising out of or related to participation in the camp, including but not limited to: • Personal injury • Illness or medical conditions • Property damage or loss • Negligence claims • Accidents during camp activities • Transportation related incidents • Financial claims or reimbursement demands • Emotional distress claims • Any unforeseen incident occurring before, during, or after camp participation This release applies to all claims whether caused by negligence or otherwise, to the fullest extent permitted by law. Financial Responsibility Waiver The participant and parent/legal guardian acknowledge and agree that neither Avery Edwards Impact Camp, Home Team Apparel, Avery Edwards, nor any affiliated party shall be financially responsible for any injuries, damages, losses, medical bills, therapy costs, rehabilitation expenses, lost wages, future care, or other personal or business related expenses arising from participation in the event. The participant and parent/legal guardian further agree not to pursue or impose any personal, business, insurance, legal, or financial liability against Avery Edwards Impact Camp, Home Team Apparel, Avery Edwards, camp staff, volunteers, sponsors, or affiliates. Conduct & Removal Camp organizers reserve the right to remove any participant from the event without refund for unsafe behavior, misconduct, bullying, fighting, inappropriate language, or failure to follow camp instructions and safety guidelines. Photo & Media Release The participant and parent/legal guardian grant Avery Edwards Impact Camp and Home Team Apparel permission to use photographs, videos, audio recordings, social media content, and other media captured during the event for promotional, marketing, advertising, and commercial purposes without compensation or further approval. Refund Policy All registrations and payments are final. No refunds will be issued unless the event is canceled by the organizers. Agreement & Acknowledgment By signing below and/or submitting the registration form, the participant and parent/legal guardian acknowledge that they have carefully read, fully understand, and voluntarily agree to this Waiver, Release of Liability, and Assumption of Risk Agreement. They further understand that this agreement is intended to be as broad and inclusive as permitted by applicable law and applies to all camp related activities.
